A case manager is a client advocate, working with individuals and their support teams to develop a plan that identifies personal preferences, needs and goals. To do this, a case manager focuses on four key areas - advocacy, services, resources and funding - to help ensure all the pieces are in place to maximize lifestyle choices.

Advocacy

As an advocate, a case manager will:

* Guide you through the maze of services. * Assist you in understanding your rights and responsibilities. * Assist you in living your preferred lifestyle. * Attend meetings with providers to advocate for your rights and needed services. * Report any suspected abuse, neglect or exploitation to the Kansas Department of Social and Rehabilitation Services.

Services In finding the right service to meet your need, a case manager will:

* Assist you in locating services. * Assist you in accessing services. * Monitor services to ensure they continue to meet your needs. * Assist you in communicating with service providers. * Assist you in changing service providers when needed.

Resources As a resource tool, a case manager will:

* Provide you with information about resources available to you. * Know where to look for the resources you need.

Funding In helping you secure appropriate funding, a case manager will:

* Have a basic knowledge of your funding systems. * Know whom to contact when funding problems arise. * Be able to assist providers with funding problems.
