Case Management

Coordinating services that assist individuals with disabilities in achieving and maintaining their preferred lifestyle

CPRF case managers assist individuals and their support network, to identify, obtain and coordinate services and supports to enhance the person’s independence. One of the most important tools a case manager uses toward assisting a customer is a Person Centered Support Plan, developed to identify the individual’s preferences, needs and goals.

Targeted case management is available for people who meet the Kansas Mental Retardation/Development Disability eligibility criteria.
